Telehealth Consent

1. Purpose

This Telehealth Informed Consent ("Consent") describes the terms under which you may receive healthcare services through telehealth technology via the Nuv platform (a HealthFare Group company). By proceeding with a telehealth consultation, you acknowledge that you have read, understand, and agree to the terms outlined below.

2. What is Telehealth?

Telehealth involves the delivery of healthcare services using electronic communications, including video conferencing, secure messaging, and other digital technologies. Through telehealth, your Provider can diagnose, consult, treat, and prescribe medications without an in-person visit.

3. Providers

Medical services are provided by independently licensed healthcare professionals, including physicians (MD/DO), nurse practitioners (NP), and physician assistants (PA), who are licensed in the state(s) where you receive care. The primary provider overseeing treatments is Andrew Joseph Hewitt, a doctor with extensive patient care experience.

All clinical decisions are made independently by your treating Provider based on their professional medical judgment. Nuv has financial relationships with the entities that employ or contract with Providers who deliver services through the Platform.

Off-Label and Investigational Use

Some peptide therapies, including BPC-157, are used in an off-label or investigational capacity. These peptides have not been approved by the FDA for specific medical indications. Your Provider will discuss the evidence, potential benefits, and risks of any off-label treatment before prescribing. You have the right to decline any treatment.

7. Compounding Pharmacy

Medications may be compounded (custom-prepared) by Strive Pharmacy, a licensed compounding pharmacy that follows strict quality standards. Compounded medications are not FDA-approved products; however, they are prepared using FDA-approved ingredients in accordance with applicable state and federal pharmacy regulations (503A/503B).

You are free to obtain your prescription from any pharmacy of your choice. Prescriptions may be transferred upon request.

8. Emergency Situations

Telehealth is not appropriate for medical emergencies. If you experience a life-threatening condition, call 911 or go to the nearest emergency room immediately. Examples include chest pain, difficulty breathing, severe allergic reactions, or signs of stroke.

All telehealth consultations are conducted through HIPAA-compliant platforms. Your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. For more information, see our Privacy Policy.

11. No Insurance Billing

Nuv operates on a cash-pay model. Services are not billed to insurance, Medicare, or Medicaid. You are responsible for the full cost of all consultations and medications.
